Participants are allowed membership and participation with only one group purchasing organization (GPO) in a designated therapeutic area. By completing the Letter of Commitment (LOC), participant informs Genentech of the participant's most current GPO affiliation for Genentech products in the designated therapeutic area.

Form Submission Process

●	This form requires an attestation by an individual who has signing authority for the participant and, where applicable, any of its wholly owned sites.
●	Once complete, you will receive an auto-generated email confirming the receipt of your submission.
●	Your submission will be reviewed within 3 business days from the date it was received.

Reminders

●	A separate LOC, where applicable, must be submitted for each therapeutic area.
●	All LOC submissions are subject to review and approval by Genentech.
●	LOC submissions are subject to certain limitations and conditions.
